Accident Report Database

The Plymouth Police Department offers accident reports in several formats.

• Online download from CRASHDOCS.org
• You may obtain a crash report by contacting us directly.
• You may E-mail the State of Wisconsin Department of Transportation at: traffic-accidents.dmv@dot.state.wi.us or, and you may contact them Monday thru Friday, 7:30 A.M. to 4:30 P.M. at 608-266-8753. You will need the following information: The “DOT Document Number” printed on the upper right corner of the Wisconsin Motor Vehicle Driver Exchange of Crash Information sheet given to you by the officer, or the date of the accident and the name and driver’s license number of one of the drivers. Requests may take up to 30 days to be received from the State.
• You can request a copy of the report from your insurance agency if they have already obtained it from us.
